Eligibility

  • Within 21 days: Free registration.
  • After 21 days – within 1 year: Late fee + written permission from authority.

How to Apply Online for Birth Certificate in Assam (Sewa Setu Portal) ?


  1. Visit the official portal: https://sewasetu.assam.gov.in
  2. Login using your registered mobile number or create a new account.
  3. In the search bar, type “Birth Certificate” and select the service.
  4. Click on Apply and fill in details – child’s name, date of birth, place of birth, parents’ details, and address.
  5. Upload scanned copies of the required documents.
  6. Review the form and submit it.
  7. Note down the Application Reference Number for tracking.
  8. You can track status via the “Track Application” option on the portal.
  9. Once approved, download the certificate or collect it from the issuing authority.

Offline Application Process


  1. Visit your local Municipal Board / Town Committee / Panchayat Office.
  2. Get the Birth Certificate application form from the Registrar of Births and Deaths.
  3. Fill it with accurate details and attach required documents.
  4. Submit to the registrar and collect receipt/acknowledgement.
  5. Collect your certificate on the given date.
