Eligibility

How to Apply Online for Birth Certificate in Assam (Sewa Setu Portal) ?
- Visit the official portal: https://sewasetu.assam.gov.in
- Login using your registered mobile number or create a new account.
- In the search bar, type “Birth Certificate” and select the service.
- Click on Apply and fill in details – child’s name, date of birth, place of birth, parents’ details, and address.
- Upload scanned copies of the required documents.
- Review the form and submit it.
- Note down the Application Reference Number for tracking.
- You can track status via the “Track Application” option on the portal.
- Once approved, download the certificate or collect it from the issuing authority.
Offline Application Process
- Visit your local Municipal Board / Town Committee / Panchayat Office.
- Get the Birth Certificate application form from the Registrar of Births and Deaths.
- Fill it with accurate details and attach required documents.
- Submit to the registrar and collect receipt/acknowledgement.
- Collect your certificate on the given date.
